What kinds of plans are available?: We have four plan designs available for groups of 1-4 employees with individual deductibles ranging from $1,500 to $7,350 per year. Each medical coverage plan includes a nationwide provider network, enhanced ambulance coverage and $0 copays for preventive care visits.

We also offer options for larger groups with 5-500+ employees. Please reach out for specific large group options.

When is your enrollment period?: Groups can enroll in a plan through MOMENTUM AG any month of the year.

Can I keep my doctor?: Our team will work closely with you to make sure that all of the doctors and hospitals you need are included in our provider network before you sign on the dotted line. We want you to be happy with our coverage and we want you to know you have the peace of mind that you can see the providers you need to see.

How do I apply? How long will it take before I get my quote?: Click the “Get a Quote” button at the top of this page to get started. You will then receive an email with a link including instructions on how to proceed. You should receive your quote within 7-10 business days.

How are my rates determined?: Because you and your employees complete a health questionnaire, rates are based on the age, gender and health of the members in your group.

Once I get my quote, how long do I have before I have to make a decision or enroll before I would have to reapply?: Your firm quote is given to you after all applications are complete and the quote is valid for 60 days.
